The nursing profession encompasses many specialties, and those specialties all have different ways of caring for a patient. During the course of an illness, a patient can pass through many different specialties: ER, ICU, telemetry, rehab, outpatient rehab, and home health just to name a few. Within these types of nursing are other adjunct therapies provided by respiratory therapists, occupational therapists, physical therapists, etc., as well as multiple physicians from different services managing the care. One thing remains constant throughout this course of treatment- the patient. An example of this would be looking at the functional history of a patient with Parkinson’s disease. Being able to access information from a past visit to gather data on ambulatory status or if they had any type of swallowing impairment is useful in delivering appropriate care. It provides for efficient and effective communication overall and facilitates collaborative and patient centered care.
Collecting data should be done in a standardized language so that it provides meaningful information to the health care team. By having standardized language in the EMR, a nurse can streamline charting so that when reviewed by others it supports knowledge used to care for the patient. An example of this would be assessing a patient with chest pain. I work in a cardiovascular ICU. When a patient in our unit has chest pain, we document it using the same verbiage as a nurse on the medical floor with no specialty in cardiac care. Chest pain can be described in numerous ways, but using the choices provided in the charting focuses the assessment in ways that are meaningful for treatment. Another example is charting of ambulation after cardiac bypass graft surgery. Sometimes patients need to go to a rehab facility. If, in our unit, we have historically used language that designates a patient ambulating as “independent,” “does well,” or “needs help,” there is nothing meaningful in that assessment for an outside unit that lets them know if a patient can ambulate independently with a cane or walker, if they can walk only 25 feet, or 150 feet. Being able to translate data into wisdom is key in order to develop a treatment plan to meet the patient’s individual needs (Laureate, 2014: 2024 – Essay Writing Service. However, it seems that it would be most beneficial for standardization of nursing language to consist of aspects that are specific but not exclusive to specialties.
